Education Office Annual Authorization Forms

The annual authorization packet has been mailed to all parents/guardians, but the same packet is also available online for your convenience. Packets may be returned to the Education Office by mail, digitally scanned and emailed to the Education Office, or faxed securely to 781-961-0888.

Health Room News and Forms

Missing documentation can delay proper medical treatment and admittance. For your convenience, all medication orders and updated forms can be mailed or faxed to the Health Room at any time prior to the start of the school year.  The health room fax is secure and only accessed by nursing staff.  Email is also available for those who want to send scanned documents.

Preventative Dental Cleaning Opportunities

Parents/Guardians:
Please review the attached informational packet, including consent form, regarding our upcoming dental hygienist visit by Visiting Dental Associates of MA. Those who wish to participate should return the consent form to the Nurses Office, along with payment information. This is an optional service; students will not participate if we do not receive consent and payment info.
Consent forms, with payment information, can be faxed to 781-961-9393, or scanned/emailed to tracy.shepherd@bostonhigashi.org. Those who are paying privately by check can mail or hand deliver their check payment, along with the signed consent form.  To guarantee participation, consent and payment must be received  no later than OCTOBER 27th, 2023.
Be advised that VDA of MA will return in the fall (they visit approx. every 6 months), and students may participate in one or both sessions; if relying on MA Health coverage, parents are responsible for confirming eligibility with them prior to visit.
